INITIAL SESSION FORMSForms are now completed electronically. There is no need to print anything. It is all now by direct entry. You will receive an email with instructions on how to complete intake forms prior to your initial appointment. If you do not receive an email 24 hours before your appointment, please email Lisa. All forms must be complete prior to intake, or you will need to reschedule.
